1. This Form will grant you the rank of Vacation, which will preserve your rank in the server.
Things that qualify for an Accepted Form includes: Studying for finals where you can't be on
the server, Grounding, Vacation, Family Emergencies, emotional issues, Health issues which
render you in the hospital, and a Change in Job shifts.
2. Please be honest with your forms. Continuously filling out forms will result in a Demotion
3. Follow the Form and fill out EVERY LINE. Do not leave any information out
4. If you wish to keep your family emergencies to yourself and not explain, please type:
"Family Emergency: No Comment" or "Health Emergency: No Comment" if you yourself have a
Medical Emergency that you wish not to speak of.
